Anona has prepared two documents that can help you bless your family with vital personal information and your final wishes for how you want to be remembered at your death.  The Private Information Form has a place for you to record important personal information such as financial accounts and their passwords, location of other valuable documents and how you want your personal property handled.  This form is NOT to be returned to the church but instead should be kept in a safe place with copies available to your family.

The Personal Wishes form allows you to record your preferences for the handling of your earthly remains and the type of service and specifics you would like included in your service.  This form should be returned to the church office after you make copies for your family.  Planning ahead and preparing these documents is a gift you can give your loved ones and allow you peace of mind.  If you have questions or need help with the forms, please contact Gary Thomas at 595-2581 ext 225.
